That's called an Avatar.

You'll have to register as a member if you want one. Membership is free.

Yup, just look for the "register" link at the top of any page. You will also be sent an activation e-mail to allow you to log in. Then click "log in" (on the top of any page) and type in your user name and password. If you're using a computer of your own, select "Save Password" or whatever (the check box).
