Perhaps you should put some text on the upload pages stating that one should NOT upload an item unless they are the author, or have specific permission from the author to upload them.
it should also state that the author's name should be placed in the "maker" box, and not the name of the person uploading it.
I know it won't stop everyone from doing so, but it might stop a few folk who are unaware of such a procedure from uploading someone else's stuff without permission.

This happens from time to time. Surprisingly not as much as it used to. And there was a guy a few weeks ago who uploaded his own because he didn't realize we'd saved it. Still, I glanced at the upload page and might do well to add a couple lines.
